During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/24/2025. The on-site inspection began at 12:20 PM and ended at 2:10 PM. The inspector reviewed compliance in the areas listed above. There were 24 children present and 7 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 3, 2025. A POC submitted after this date will not appear on the public website.

The child day center shall obtain a central registry background check for each employee that has resided outside of the state of Virginia within the last 5 years, within the first 30 days of employment.

Staff #5 has resided outside of the state of Virginia in the last 5 years. Staff #5 record does not contain the results of an out of state background check for the state that they resided in.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

1. A spray bottle labeled "Bleach solution" and a bottle of rubbing alcohol was observed on a shelf in a cabinet in the Preschool classroom. The lock on the cabinet was not latched/broken.

Cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food

1. A bottle of bleach solution was observed stored on a top shelf in the Two's classroom. The cleaning solution was next to a zip lock bag of cereal, and over a stock of paper plates, cups and utensils.
2. A bottle of bleach solution and bottle of rubbing alcohol was observed stored on a top shelf in the Preschool classroom. Both solutions were stored above a stock of plates, bowls and utensils.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.

Child #6 emergency medication was stored inside of a medication bag, within a backpack, located in the Toddler classroom. The lock on the medication bag was broken.
