A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The records for child #4 and child #5 had blank spaces where there was an inquiry of information regarding any allergies or special care required.

Plan of Correction: Paperwork updates are being requested of the se children's parents at pickup today. Children's records will be reviewed now and upon each new enrollment to ensure parents do not leave required information blank, but write "None" or "N/A" or other appropriate indication acknowledging the inquiry.

Staff who work directly with children are required to annually attend the department's health and safety update course. Staff #2 and Staff #5 did not have documentation of completing the department's health and safety update course within the last 12 month period. Staff confirmed that the course had not been taken in the past year.
Plan of Correction: The program director has instructed staff #2 and Staff #5 that the annual health and safety update training must be completed by April 23, 2025.
