An unannounced monitoring inspection was conducted today.
There were 18 preschool children (ages 3-5yrs) present with two classroom staff.
The children were eating a snack, doing movement activities and having group time.
Three child records were reviewed.

Based on a tour of the playground, the facility failed to ensure that all areas are kept in safe condition.
Evidence:
1. On the new climber, there is a 1/4 inch gap between the slide and the deck. This presents an entanglement hazard.
2. There are three spikes protruding more than one inch from the plastic retaining material.
These spikes present a protrusion hazard.

Plan of Correction: The facility manager will fill in gaps on the climber. Children will not use the climber until it is fixed.
