Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was conducted virtually on 01/26/2022 and concluded with a virtual record review inspection on 01/27/2022. The provider was contacted by Zoom and a virtual inspection was conducted. There were 9 children, ages 5 years-11 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on discussion with staff, the center failed to develop written procedures for prevention of shaken baby syndrome or abusive head trauma.

Evidence: Staff 3 stated that the center did not have written procedure for prevention of shaken baby syndrome or abusive head trauma.
Plan of Correction: Currently working on procedures.
Standard 8VAC20-780-530-C
Based on record review and discussion with the staff, the center failed to ensure that there shall be at least two staff members who meet the requirements of subsection A of this section present on the premises during the centers during the center?s hours of operation, on fieldtrips, and wherever children are in care.

Evidence: There was only one staff member that met the requirements of subsection A.
