Inspector Notes

An unannounced monitoring inspection was conducted today, March 8, 2022. There were 10 children present with 2 care givers. The children were observed playing with floor toys nd playing outside. The physical plant, tuberculosis tests, children's records, caregiver's and household member's records, attendance records, evacuation logs, medications and menu's were reviewed. Based on a review of children's records, the provider failed to obtain all required documentation for all children enrolled.

Evidence:
Child #3's record does not have documentation of a work phone number for mother of child.
Child #4's record does not have documentation of a work phone number or work address for father of child.
Child #4's record does not have documentation for an address for one of the emergency contacts.
Child #'s record does not have documentation of a medical insurance policy number.
Plan of Correction: I will request this information from the parents.
