Inspector Notes

An unannounced renewal inspection was conducted today, on July 21, 2021. There were 11 children present (27 points) with 2 caregivers. The physical space, children's records, caregiver records, household member records, first aid supplies, menu's, cots, refrigerator temperature, training logs, evacuation logs, medication and medication authorizations were reviewed. The children were observed playing floor game, singing songs and working on art activities. Areas of non-compliance were cited on the Violation Notice. Based on a review of household member records, the provider failed to request a fingerprint background check for resident of Family Day Home that reached the age of 18,

Evidence:
Household Member #3's record did not have documentation of a criminal history/fingerprint background check. (DOB-05/03/2004)
Plan of Correction: I will submit the request for fingerprints as soon as possible.

Based on a review of household member records, the provider failed to ensure that when a person in the home becomes 18 years of age a sworn statement is completed.

Evidence:
Household Member #3 record does not have documentation of a sworn statement.
Plan of Correction: I will have the household member to sign a sworn statement today.
