During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on March 17, 2025 and completed on March 17, 2025. The on-site inspection began at 1:10pm and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records [on-site].
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 25, 2025. A POC submitted after this date will not appear on the public website.
Violations
8Staff 1's record did not contain a sworn statement or affirmation.
At the beginning of the inspection all staff members were caring for children and counted in ratio. There was not an additional staff member immediately available. Staff 4 arrived at the center from picking up school age children from school approximately twenty minutes after the start of th inspection.
In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.
At the beginning of the inspection Staff1 was the sole care provider in Classroom A. Staff 1 does not meet the qualifications of a program leader.
The center shall maintain a ratio of one staff for every four infants under the age of sixteen months.
At the beginning of the inspection the Classroom A had a ratio of one staff for seven infants. A second teacher arrived to the classroom after approximately ten minutes.
The center did not follow it's safe sleep policy. Four infant were observed sleeping in bouncy chairs.
An infant was observed drinking from a bottle propped by a stuffed animal while laying on a floor pillow.
Children's records shall contain all required information.
Child 5's record did not contain the name, address, and phone number for a second emergency contact.
Staff records shall be maintained with all required information.
Staff 1's record did not contain documentation that at least two references were checked.
