During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated on March 3, 2026 and completed on March 11, 2026, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as a result of the last inspection related to: staff records, tuberculosis screenings, and staff orientation. The on-site inspection began at 11:30am and ended at 12:00pm on March 3, 2026 and began at 11:15am and ended at 11:50am on March 11, 2026. The inspector reviewed compliance in the areas listed above. The inspector reviewed 4 staff records on-site and electronically on March 4, 2026. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been
corrected. Submit your POC within five business days from today, which will be the close of business on March 30, 2026. A POC submitted after this date will not appear on the public website.

5On March 4, 2026, Staff 1 submitted electronic staff records for review. Staff 2?s record showed seven documents with altered dates. In an interview on March 11, 2026, Staff 1 stated they had added dates and later changed dates on Staff 2?s paperwork, even though those dates were supposed to be completed by Staff 2.

The orientation training for Staff 2 is documented as being completed eight days after employment.
On March 11, 2026, Staff 6 left Classroom A to go to the kitchen for approximately five minutes, leaving one staff member supervising seven children aged 16 to 24 months.
