During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on10/23/2024 and completed on 10/23/2024 in response to a complaint received by the licensing office on 10/18/2024 relating to allegations of staff qualifications, staffing, and terms of license. The inspector reviewed one child?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, attended the facility for three months before the center had an immunization record on file.
Plan of Correction: The parents of child #1 will be advised to provide immunization records. In the future the administration will ensure no child starts at the center until immunizations are received.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, who had been enrolled for seven months.
Plan of Correction: The parents of child #1 will be advised to provide a physical. In the future the administration will ensure physicals are obtained in the required timeframe.
