An unannounced monitoring inspection was initiated on March 19, 2025 and completed on March 19, 2025 in response to a self-report received by the licensing office on March 19, 2025 relating to behavioral management and forbidden actions. The inspector reviewed one staff record on-site. This inspection included interviews and document review. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
