During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 5, 2026 and completed on March 5, 2026. The on-site inspection began at 10:30 a.m. and ended at 2:15 p.m. The inspector reviewed compliance in the areas listed above. There were 70 children present and 12 staff. The inspector reviewed seven children?s records and five staff records on-site. This inspection included a tour of the facility, record review, observations, and interviews.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 13, 2026. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-A
Pending the results of all background check components, an applicant for employment may work in the child day center, provided that (a) the applicant has received qualifying results on a fingerprint-based background check through the Central Criminal Records Exchange or the Federal Bureau of Investigation and (b) the applicant is supervised at all times by a person who received a qualifying result on a background check conducted within the past five years.
Staff #1 was alone with children during the inspection and the results of the central registry background checks had not been received.
Plan of Correction: Staff will be with an employee until we receive central registry unit results.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The record for child #3, age 14 months, was missing the one year immunization record. The record for child #5, age 7 months, did not have a six month immunization record.
Plan of Correction: Records have been fixed and updated.
Standard 8VAC20-780-500-B
The diapering area shall have a nonabsorbent surface for diapering or changing. The diapering surface in the Elephant room was torn in two places exposing the absorbent sponge.
Plan of Correction: Grabbed new one same day of inspection from our head quarters.
Standard 8VAC20-780-550-E
Shelter in place procedures shall be practiced a minimum of twice per year. Per documentation only one shelter in place drill was conducted in 2025.
Plan of Correction: Shelter-in-place will be done this month.
