During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An announced inspection was initiated on February 5, 2025 and completed on February 5, 2025 with the local Child Protective Services, in response to a complaint received by the licensing office on January 30, 2025 relating to allegations of supervision. The inspector reviewed 1 child?s record and 4 staff records [on-site]. This inspection included document review, tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 19, 2025. A POC submitted after this date will not appear on the public website.

5On January 29, 2025 Staff 1 was the sole staff member assigned to Classroom A in the morning. Staff 1 does not meet the qualifications of a program lead.

1. On January 29, 2025 Child 1 (4 years old) was left in a publicly accessible restroom for approximately seventeen minutes. At the time of the incident Staff 1 was supervising a group of children from outside of the restroom. Staff 1 then left to go to a scheduled activity with the group. Child 1 was inadvertently left in the restroom while Staff 1 and the other children transitioned to another activity.
2. Child 1 was alone for approximately seventeen minutes when a building employee found them crying and escorted them back to the Child Day Center.

Child 1 was unaccounted for on January 29, 2025 from approximately 10:03am until 10:20am. Center management was made aware of the incident at 10:25am. The parent was notified at 2:06pm, approximately one hour and forty-five minutes after the child had been picked up for the day.
