During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on August 5, 2025 and completed on August 25, 2025, as a part of the provisional licensure period. The on-site inspection began at 10:15am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 106 children present and 22 staff. The inspector reviewed 10 children?s records and 5 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 12, 205. A POC submitted after this date will not appear on the public website.

Toys or objects hung over an infant in a crib and crib gyms that are strung across the crib may not be used for infants over five months of age or infants who are able to push up on their hands and knees.

A sleeping outfit was observed draped over the rail of an occupied crib above an infant's head.
Plan of Correction: The sleep sack was immediately removed and we discussed the importance of not draping items on cribs. Routine checks will be performed to ensure this doesn't happen again.

Staff and children's records shall be treated confidentially.

Confidential information about a child was posted on a board in Classroom A.
Plan of Correction: This confidential information was immediately removed and a reminder was given to teaches about keeping confidential information off the boards.

Children's records shall be maintained with all required information.

Child 3's record did not contain a second emergency contact.
Plan of Correction: We will obtain a second emergency contact for this child.
