Inspector Notes

This inspection was conducted by licensing staff, in part by using an alternate remote protocol, including telephone contacts, documents review and interviews. An in-person tour of the program was also conducted
A Renewal inspection was initiated on 2/9/2022 and concluded on 2/11/2022. The provider was contacted by telephone and a virtual inspection was conducted. The inspector also completed an "in-person" component of the inspection on site. On 2/11/2022 there were 22 children present with 4 staff during the site inspection. Three staff records and three children?s records were reviewed. The inspector reviewed requirements and compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the licensee was unable to demonstrate that the results of a
Search of central registry is obtained for any agent who is or will be involved in the day-to-day operations of the facility.
Evidence:
The licensee was unable to locate documentation that a search of the central registry ahd been obtained for Agent #1.
Plan of Correction: The central registry results were done previously but the licensee was not able to locate the results at the time of the inspection. Documentation was provided to the licensing inspector that a new request for Search of the Central Registry for Agent #1 had been submitted and paid for on 2/11/2022. The results will be forwarded to the inspector upon receipt.
