Inspector Notes

A monitoring inspection was initiated on 6/6/2023 and concluded on 6/7/2023.? There were 29 children present with 6 staff supervising. The children were observed playing outside, eating snack, and during check out time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 4 pm and concluded at 5:55 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview with staff, the center did not ensure to obtain a fingerprint based national criminal record check prior to the first day of employment.
Evidence: The director confirmed the following staff did not have a criminal record check prior to their first day of working in a supervisory capacity
1. Staff #2 criminal record check was dated 6-15-2021 and their first day was 5-21-2021
2. Staff #4 criminal record check was dated 6-15-2021 and their first day was 6-13-2021
3. Staff #5 criminal record check was dated 7-28-2021 and their first day was 7-13-2021
Plan of Correction: We will make sure all documents are completed before a staff's first working day in the future.
Standard 8VAC20-770-40-D-1-b
Based on record review and staff interview the licensee did not ensure that an employee who is alone with, in control of, or supervising one or more children shall have a sworn statement prior to their first day of employment.
Evidence: The director confirmed the following staff did not have a sworn statement prior to their first day of working in a supervisory capacity
1. Staff #2 sworn statement was dated 5-23-2021 and their first day was 5-21-2021
2. Staff #5 sworn statement was dated 7-14-2021 and their first day was 7-13-2021
Plan of Correction: We will make sure sworn statements for future staff are completed prior to first day of employment.
Standard 8VAC20-780-160-A
Based on record review and staff interview the licensee did not ensure documentation of the Tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: Staff confirmed that Staff #4?s TB screening was dated 7-21-2021 and the first date of employment was 5-21-2021.
Plan of Correction: We will make sure that future TB results are given before coming into contact with children.

Based on record review and staff interview the licensee did not ensure to include methods to prevent use of outdated medication
Evidence: Staff confirmed that child #6 Epinephrine Auto-inject medication expiration date was 3-30-2023 and was expired.
Plan of Correction: Mother of child has replaced with new epi-pen.
