Inspector Notes

An unannounced inspection was initiated and completed on September 9, 2024 in response to a complaint received by the licensing office on August 27, 2024 relating to allegations of physical plant, staffing/supervision, programs, special care provisions and special services. This inspection included tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 19, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Based on observation, facility did not ensure children under 10 years are always within sight and sound supervision of staff.

Evidence: At approximately 11:50 am, staff in the 2's classroom were observed assisting children inside the bathroom and 7 children were observed running freely on the other side of a 1/2 wall in the classroom. The 7 children were not in sight supervision of staff.
Plan of Correction: Staff were met with and reminded that all children should be within sight and sound.
Standard 8VAC20-780-560-J
Based on interviews, facility did not ensure tables are sanitized before and after eating.

Evidence: Staff in 2 of 3 classrooms stated they wipe the tables with disinfectant wipes before children eat meals and snacks. Disinfectant wipes do not meet the definition of sanitized as the surface was not sprayed or dipped with a disinfectant solution and allowed to air dry.
Plan of Correction: Bleach water is now used to follow cleaning for sanitation.
