A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was due 7 months ago the updated screening has not been completed.

Hazardous substances must be in a locked place that prevents access by children. In the two year old bathroom, toilet bowl cleaner was stored on a open shelf, out of reach of children.
Plan of Correction: Cleaner was moved to a locked cabinet.

Playground equipment with moving parts or climbing apparatus must have appropriate resilient surfacing depth in the fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. There was approximately 2" depth of mulch at the foot of the two slides and green circular climber, where there should be at least 6" depth.
Plan of Correction: The climbing equipment on the playground will not be used until sufficient mulch is added.
