Inspector Notes
An annual monitoring inspection was initiated on 02/13/2023 and concluded on 02/13/2023. The inspector was on site at the center from 10:15am to 12:40pm. There were 12 children in care with 2 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and all classrooms were inspected today. Five children?s records and six employee records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

1. The record of staff #1 (employment date: 7/5/22) did not have documentation showing that they have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
2. The record of staff #5 (employment date: 3/18/21) did not have documentation showing that they have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
3. The record of staff #6 (employment date: 7/5/22) did not have documentation showing that they have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Evidence:
During the inspection of six staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years.

1. The record of staff #1 (employment date: 7/5/2022) had documentation of fingerprint based national criminal history search results dated 7/7/2022.
2. The record of staff #3 (employment date: 11/14/2022) had documentation of fingerprint based national criminal history search results dated 2/6/2023.
3. The record of staff #4 (employment date: 3/23/2022) had documentation of fingerprint based national criminal history search results dated 4/6/2022.
Evidence:
1. The record of staff #1 (employment date: 7/5/2022) did not have documentation of criminal history record results, sex offender registry check results and a child abuse and neglect search request from any state in which the individual has resided in the past five years.
2. The record of staff #1 and an interview, indicated that the staff member has lived outside of Virginia within the last 5 years.
Evidence:
The licensing inspector did not see a carbon monoxide detector installed in the center. The center?s administrator stated that a carbon monoxide detector is not currently installed in the building.
