Prior to employment, and annually thereafter, each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
1. Staff #3, employed for 3 months, did not obtain a staff health report.
2. The annual staff health report for staff #4 was overdue by 2 months.

5. The annual staff health report for staff #10 was overdue by 2 years and 9 months.

1. Staff #1, who has been employed for 7 months, did not have the results of a search of the central registry. Staff #1 was working alone in a classroom with children.
2. Staff #3, who has been employed for 3 months, did not have the results of a search of the central registry. Staff #3, was left alone in the classroom with children several times during the inspection.

2. For staff #9, the e-background check has been requested and completed and will be kept on file in their record. The paper request is being mailed in once the employee signs the form. It will be sent certified with return receipt requested and the tracking information will be kept in her file until the background check comes back.

Plan of Correction: We have developed a form for parents to include in each child's file asking if they have or have not attended another school previously and if they have, which school(s) have they attended. These forms will be given to all parents, current and in the future, to include in student files.
