Inspector Notes
An unannounced monitoring inspection was conducted onsite on 10/27/2022. There were 35 children, ages 4 years- 12 years, and 6 staff members providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies, special services, and background checks. The children were observed during arrival, meal time and outdoor time. A total of 5 children?s records, and 5 staff records were reviewed. The inspector discussed the following with the provider: most recent inspection, allergies, and health inspection. The inspector arrived for the onsite inspection at 3:15 pm and departed at 6:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

1. The record for Staff 1 contained a documented re-hire date of 05/16/2022. Staff 1 confirmed they left the employment of the provider approximately 04/2020 and there was not an employment eligibility letter available for review.
Evidence: The record for Staff 1 contained a documented re-hire date of 05/16/2022. Staff 1 confirmed they left the employment of the provider approximately 04/2020 and there were no findings from a search of the central registry available for review.
Evidence: There was approximately 1 inch of resilient surfacing in the fall zone of the climbing equipment. The measurement was taken at the bottom of the slide facing the building. The inspector observed a preschool age child using this equipment.
Evidence: The inspector observed approximately 10 book bags lying on the floor next to the door
Evidence: Children were observed arriving to program and immediately eating their afternoon snack without washing hands before eating.
Evidence: The record for Child 3 did not contain the following information: name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (addresses for both designated people were missing)
