Inspector Notes

An unannounced renewal inspection was conducted on 6/27/2023. There were 25 children, ages 5 years- 11 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, nutrition and background checks. The children were observed during mealtime, self-directed activities, teacher directed activities and outdoor time. A total of 5 children?s records, and 5 staff records were reviewed. The inspector arrived for the inspection at 1:15 pm and departed at 3:15 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice to the provider.

Based on record review, the center failed to obtain documentation that each child has received the immunization required by the State Board of Health before the child can attend the center.

Evidence: The record for Child 3 documented the first date of attendance as 05/31/2022. The immunization record was dated 08/19/2022.
