Inspector Notes

An unannounced renewal inspection was conducted on 6/1/2023. There were 7 children, ages 4 years- 10 years, and 2 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, nutrition and background checks. The children were observed during mealtime, self-directed activities and teacher directed activities. A total of 5 children?s records, and 2 staff records were reviewed. The inspector arrived for the inspection at 9:30 am and departed at 11:30 am.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice to the provider.

Based on observations and interviews with staff, the center failed to ensure that all employees shall undergo a background check in accordance with the Code of Virginia prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence:
1. The most recent sworn statement for Staff 1 was dated 12/28/2017. Staff 1 and 3 confirmed there was no 2022 sworn statement available for review during the inspection.
2. The most recent search of the central registry for Staff 1 was dated 07/13/2017. Staff 1 and 3 confirmed there was no 2022 search of the central registry available for review during the inspection.

Based on record review, the center failed to implement a monthly practice evacuation drill.
Evidence: There was not documentation that a monthly practice evacuation drill took place in November 2022, December 2022, January 2023 or February 2023.

Based on the record review, the center failed to ensure that shelter in place procedures shall be practiced a minimum of twice per year.
