Inspector Notes

An unannounced monitoring inspection was conducted on 12/7/2023 from 12:00 pm to 2:10 pm There were 94 children (infants-5yrs.) supervised by 16 direct care staff. The physical plant, 5 staff records, 8 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. REPEAT VIOLATION

Based on observation and interview, areas and equipment of the center, inside and outside, were not maintained in a clean, safe, and operable condition

Evidence:
1) There was a separation in the seam of the resilient surfacing near the yellow bars, treehouse, and seesaw, causing a tripping hazard on the playground.

2) There was a few areas of peeling paint on the doorframe of the playground's door.
