Evidence:
1. In the 4's classroom, a container of multi-surface disinfecting wipes was found on the classroom cubbies.
2. Staff #7 stated that the multi-surface disinfectant wipes were for Child #11, whose parents brought them in to clean the child's hands.

Plan of Correction: Sending it off this afternoon.
Standard 8VAC20-780-60-A
Based on record review and interview, the center did not obtain all the required information for each child's record at least annually.

Evidence: Child #5's last annual update confirming the child's information in the child's record was up-to-date was 9/29/22. The start date for Child #5 is 10/3/2022.
Plan of Correction: I will have them check the emergency contacts tonight. We check and update everyone in the summer.
