Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A complaint inspection was initiated on September 2, 2021 and concluded on September 15, 2021. A complaint incident was received by the department regarding allegations in the area of staffing and supervision. The director was contacted by telephone and a virtual inspection was conducted. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation did not support the allegation of staffing and supervision. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
1Evidence: 1. Injury report #1 did not contain documentation of how the parent was notified of the injury. 2. Injury reports #3 and #5 did not contain documentation of the time the parent was notified of the injury and any future action to prevent recurrence of the injury. 3. Administration acknowledged the written reports weren't complete.
