Inspector Notes

An unannounced monitoring inspection was conducted on Monday, October 30, 2023. The inspection began at 9:00am and concluded at 12:00pm. There were 33 children present, ranging in ages from 8 months to 4 years, with 7 staff supervising. The inspector observed children participating in circle time, and completing a coloring worksheet. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, transportation, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-240-I
Based on interview and review of staff records, the center did not obtain documentation of orientation training for a staff member.
Evidence:
1. The record for staff 5 with a hire date of 7/24/2023 completed orientation training but did not have record of the training.
Plan of Correction: Will have staff update.
Standard 8VAC20-780-270-A
(Repeat Violation) Based on observation and interview, the applicant did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
1. Two of the center?s buses were inspected. Multiple areas on the seats were observed where the vinyl was cracked and torn, exposing the foam cushioning.
Plan of Correction: The bus seats will be taped.
Standard 8VAC20-780-280-B
(Repeat Violations) Based on a tour of the physical plant, the facility failed to ensure that all hazardous substances are kept in a locked place.
Evidence:
1. In the two's classroom, a cabinet in the bathroom was not locked. There was a can of Lysol Disinfectant Spray, and Purcell Professional Surface Disinfectant.
Plan of Correction: Church personnel will dix cabinet lock.

(Repeat Violation) Based on record review, the center did not ensure that children?s records were complete.
Evidence:
1. The record for child 1 with an enrollment date of 3/31/2014, did not contain documentation of an annual update.
2. The record for child 2 with an enrollment date of 10/21/2019, did not contain documentation of an annual update.
3. The record for child 3 with an enrollment date of 7/17/2023, did not contain documentation of parent contact information, the parent?s work contact, and the child?s physician contact information.
Plan of Correction: Notified parents will be corrected upon pickup.
