Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on 09/14/2021 and concluded on 09/23/2021. There were 54 children present with four staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee failed to ensure that documentation was completed within 30 days prior to the staff date of employment for results of a tuberculosis test or screening.

Evidence:
1. Staff #2 had a documented hire date of 9/16/2021. The TB test/screening was dated 6/11/2021.

2. Staff #4 had a documented hire date of 9/16/2021. The TB test/screening was dated 1/21/2021.
Plan of Correction: The administrative staff will ensure that TB tests or screenings have been completed within the last 30 days prior to hire.
