Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 2/24/22 and concluded on 3/11/22. The director was contacted by telephone and a virtual inspection was conducted. There were 29 children present, ranging in ages from five years to ten years, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed.

Based on an interview with the program director and a review of documents, the center failed to ensure that all persons who indicate on a sworn statement having lived outside of Virginia in the last five years, and who work alone with children, have completed all required background checks prior to employment.
1. There was no out-of-state sex offender background check conducted for staff #2 prior to the hire date of 12/13/21.
2. There was no out-of-state criminal history name check conducted for staff #2 prior to the hire date of 12/13/21.
3. There was no follow-up on a out-of-state child abuse and neglect search 45 days after sending the request to search for staff #2 with a hire date of 12/13/21.
Plan of Correction: The program will ensure this information is requested and received prior to employment.
