Inspector Notes

An unannounced mandated renewal inspection was conducted on 04/08/2024. The inspector was on site from 2:20pm until 3:15pm.

Inspector reviewed four children?s record, two medical records and two staff member records.
After review with LA inspection documents were corrected on 5/1/2024 to correct language errors.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review of medication, the center failed to ensure medication was labeled with child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Prescription medication for Child #5 was observed in a plastic bag in the medication box. There was no label on the medication.
