The center must establish and implement procedures for the appropriate supervision of
children in care.
In or around September of 2025, Child #1 was inappropriately touched by another child while unsupervised in the preschool bathroom. On a separate occasion, witnesses stated that unsupervised children had been observed playing in the bathroom, and teaching staff were notified. The center administration stated that their policies included staff being stationed in locations that enabled them to be able to effectively supervise and keep the children safe.

Plan of Correction: 1. The center will continue to reiterate our policies with employees including the importance of compliance for the safety of the children.
2. Restroom procedures will be written separately for employees to review and sign off on.
3. The steps of communication will also be reviewed for times when additional coverage is needed for emergency situations: potty related accidents, sickness, injury etc.
Restroom policies will be compiled into one document that each employee will review and sign off on. Within 90 days, all Current Employees and New Hires will be in compliance with this written initiative.
