Inspector Notes

A virtual complaint inspection was initiated on November 24, 2021 and concluded on November 29, 2021. A complaint was received in the Valley Office of Child Care Health & Safety on November 23, 2021 regarding a serious injury and emergency procedures. The director was contacted by telephone to initiate the investigation. Two staff were interviewed, video footage dated 11/23/2021, one child record, onw injury report, and sign in/sign-out sheets were requested for review.

The preponderance of the evidence gathered during the investigation does not support the allegation and the complaint disposition was, therefore, "not valid". There were no violations of the licensing standards.
