Inspector Notes
A monitoring inspection was initiated and concluded on 6/14/2022. There were a total of 10 children with 3 staff members present. Children were observed resting, handwashing, diaper changing and transition to snack. Staff were actively engaged and interactive with the children.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child record and 1 household record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. Household member #1 did have a documentation of results of a criminal history record and search of a criminal history record information check. The household member moved in the home March 2022.
Evidence:
1. Household member #1 did not have documentation of a signed sworn disclosure statement. Provider stated the household member previous resided in California within the last 5 years.
Evidence:
1. Household member did not have documentation of a current Report of Tuberculosis Screening.
