Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/29/2024. The on-site inspection began at 12:15 pm and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 9 staff. The inspector reviewed 5 children's records on-site and 5 staff records electronically on 7/30/2024. The inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/6/2024. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks every five years. The most recent sworn disclosure statement for staff #1 was dated 5/17/2019 and the most recent criminal record check for staff #1 was dated 5/30/2019.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #5 had no documentation and had been employed for 2 years.

A tuberculosis (TB) screening is required every two years from the last screening. Staff #3 updated screening was due 11 days ago. The updated screening has not yet been completed.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #3 has not yet completed this course which is overdue by 13 months.
