Inspector Notes

An unannounced, on-site focused inspection was initiated on 06/13/2025 and completed on 06/13/2025, in response to a self-reported incident received in our office on 6/12/2025. The on-site inspection began at 9:45AM and ended at 10:15AM. The inspector reviewed compliance in the areas listed above. There were 34 children present and 6 staff. The inspection included electronic document review, a tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/23/2025. A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
Staff A left Child A & Child B out of sight and sound supervision, in a school hallway, for approximately 3-5 minutes while taking another group of children back to their classroom.
Plan of Correction: Staff will always ensure the sight and sound supervision of children. Staff A will attend training on SACC program administration on June 16, 2025. The regional manager will conduct a review of Active and Positive Supervision on 6/23/25. The review will include strategies for proactive communication and working through a variety of scenarios to ensure appropriate supervision.
