Inspector Notes

An unannounced renewal inspection was conducted from 3:00pm-4:15pm. There were a total of 31 children and three staff members present. Children were observed arriving to the program, washing hands, eating snack, and table top activities. A selection of staff trainings, children records, emergency plan and emergency evacuation drills were reviewed. No medications were on site this date.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's(Date of enrollment-11/1/2021) and Child #4's(Date of enrollment-11/01/2021) record did not contain the name and phone number of the child's physician.
Plan of Correction: The supervisor will obtain this information the name and phone number of the children's physician and update the records.
