Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 3:30 pm- 4:45 pm. There were a total of 40 children with three staff members present. Children were observed during snack time, handwashing, bathroom breaks, and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. 2Evidence:
1. The sofa located in the older SACC classroom had a one inch hole located in the middle of cushion exposing the foam and stuffing. The cushion material was also cracked and peeling.
Evidence:
1. At the time of the inspection, the school age classroom, (ratio 1:18) contained a total of 22 children, ages 5-7 years old, in the classroom with two supervising staff. At approximately 4:35pm, Staff #4 left the classroom to pick up children arriving to the program from a bus stop located outside of the classroom. Staff #1 was left in the classroom with a total of 22 children for approximately six minutes before a ratio of 1:18 was maintained.
