Inspector Notes

An announced monitoring inspection was initiated on 3/22/2023 and completed on 3/22/2023. A review of staff personnel files was conducted by inspectors, Margaret Allworth, Mahrukh Aziz, Leah Pagala and Derek Acosta at the Pennino SACC office. Record reviews for subs and Regional Supervisors are documented with the Lane site file.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please contact the assigned licensing inspector if you have any questions.

Standard 22.1-289.035-B-2
Based on records review, the center did not results of a finger print based criminal record check for an employee prior to their first day of employment.
Evidence:
1. Staff # 1's first day of employment was 1/6/20.
2. Staff # 1's fingerprint results were dated 1/10/20.
Plan of Correction: Moving forward all background checks will be obtained on time.
Standard 22.1-289.035-B-4
Based on records review, the center did not obtain results of an out of state central registry search for an employee that resided outside of Virginia, within 45 days of employment.
Evidence:
1. Staff # 1's first day of employment was 1/6/20. Staff # 1's record indicated that she resided in Florida within the last 5 years.
2. Staff # 1's central registry search for Florida was dated 3/14/23. It was not obtained within 45 days of employment.
Plan of Correction: Moving forward all background checks will be obtained on time.

Based on records review, the center did not obtain a complete sworn disclosure statement (SDS) for an employee prior to their first day of employment.
Evidence:
1. Staff # 1's first day of employment was 1/6/20.
2. Staff # 1's SDS was dated 1/10/20.
