Inspector Notes

An unannounced monitoring inspection was initiated and completed on April 29, 2025 in response to a self-report received by the licensing office on April 28, 2025 relating to supervision. The inspector reviewed one child's record and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May . 21, 2025. A POC submitted after this date will not appear on the public website.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by the standards.

On Friday, April 11, 2025, Child #1 was not signed out for departure in accordance with the sign out procedures outlined in the Operations Manual of the School Age Child Care (SACC) Program. The Sign Out procedure states that "Children are not permitted to go home alone. They must be picked up and signed out by an authorized pick-up person as indicated on the child's Licensing Information form". Bullet #3 states "Ask the person who picks up the child to move the child's magnet to "out" and to sign the board". The authorized pick up person for Child #1 did not enter the facility and did not move Child #1's magnet to "out" and did not sign the board. Staff #1 and Staff #2 confirmed that the Sign Out procedure was not being implemented at the facility to date.
Plan of Correction: Crestwood SACC has put in the following to ensure the center's policies and procedures are maintained in compliance with the standards. We have adjusted the way parents are greeted and welcomed into the center. Instead of greeting them at the door to SACC, Room 1 they will be buzzed in from our electronic doorbell system. From there, they will sign the child out from the parent sign out magnetic board. Their departure will also be logged on the SEMS accountability sheet and on the room 1 accountability sheet.
