Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews.

A code compliance inspection was initiated on 11-4-21 and concluded on 11-12-21. The director was contacted by telephone and a virtual inspection was conducted. There were 28 children present, ranging in ages from 5 to 12, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Based on a review of staff files, the facility failed to ensure that all staff have all required background checks.
Evidence:
1. Files for staff 3 & 4 do not contain CPS Central Registry Clearances. (hire dates 1-8-21 & 5-10-21)
Plan of Correction: Background checks have been submitted for all staff.
