Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 11/07/24 and completed on 11/07/24. The on-site inspection began at 2:40pm and ended at 4:05pm. The inspector reviewed compliance in the areas listed above. There were 54 children present with 7 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

Fingerprint results must be obtained prior to hire.

Staff #3 who was hired more than 4 months ago and has actively been working with the children did not have a completed finger print result. Administration confirmed that the fingerprints had not been completed as required.
Plan of Correction: This will be completed as required and the results placed in the staff members record for review. If this cannot be completed the staff will not be employed until completion occurs.

Results of a search of the Central Registry must be obtained on staff.

Staff #4 who was hired more than 10 months ago did not have the results of the Central registry search as required. The staff has been actively working since hire.
Plan of Correction: We will follow up with the agency to see the status of this request. If we need to submit it again we will.
