An unannounced, on-site monitoring inspection was initiated on 8/7/2024 and completed on 8/7/2024. The on-site inspection began at 11:50am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 21. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Caregiver records are to be maintained with all required information. Caregiver 1's record did not contain the results of two reference checks, the date of hire, and the name and contact information of an emergency contact.

The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
