During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site code compliance inspection was initiated on 2/10/2024 and completed on 2/10/2024. The on-site inspection began at 10:15am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 21 children present with 5 staff. The inspector reviewed 6 staff records on-site. This inspection included document review, interviews and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/28/2025. A POC submitted after this date will not appear on the public website.

3The center is required to have employees undergo a background check every five years.
The most recent national fingerprint check for staff #5 was overdue by a year and 5 months.
The provider must obtain a completed national criminal background check prior to the employee?s first day of employment.
The records for staff #1, #2, #3 and #4 did not have documentation of completed national criminal background checks. The first date of employment for staff #2 was not provided by the provider. Staff #1 has been employed for over 4 months. Staff #3 has been employed for over 1 year and a half. Staff #4 has been employed for over 6 months.
For any employee hired prior to 7/1/2024, a search of the central registry is required within 30 days of employment.
The records for staff #2, #3, #5 and #6 did not contain documentation of results of a central registry search. Staff #2, #3, #5 and #6 have been employed for at least one year.
