Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 10/06/2021, a virtual on-site inspection was conducted on 10/19/2021 and the inspection concluded with a virtual record review on 10/21/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 13 children, ages 4 years-9 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 3 children?s records, 2 staff records and 3 medications were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure that a person must not be employed until the agency has the person's completed sworn statement or affirmation.

Evidence: The record for Staff #2 had a documented hire date of 02/24/2020. The date on the sworn statement was 05/02/2020.
Plan of Correction: Completing the sworn statements are now a part of the application process.
