Inspector Notes

An unannounced monitoring inspection was conducted on 4/26/2023. There were 70 children, ages 3 years- 5 years, and 13 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The inspector discussed the following with provider: attendance, allergy list, menus and space. The children were observed in self-directed and self-chosen activities, librarian visit, outside time, mealtime and rest time. A total of 5 children?s records, 5 staff records and 5 medications were reviewed. The inspector arrived for the inspection at 9:15 am and departed at 1:15 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Standard 22.1-289.035-A
Based on observations, the center failed to ensure that all employees shall undergo a background check in accordance with the Code of Virginia prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence: The most recent employment eligibility letter for Staff 4 was dated 02/12/2018. There was no 2023 employment eligibility letter available for review during the inspection.
Plan of Correction: Staff will schedule appointment for fingerprints today.
Standard 8VAC20-780-160-A
Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and the documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The record for Staff 5 contained a documented hire date of 10/11/2022. There was no documentation of a negative tuberculosis screening available for review during the inspection.
Plan of Correction: Staff will make an appointment.
Standard 8VAC20-780-240-A
Based on record review, the center failed to ensure that all staff complete the Virginia Department of Education-sponsored orientation within 90 calendar days of hire.
Evidence: The record for Staff 5 contained a documented hire date of 10/11/2022; no documentation of Virginia Department of Education-sponsored orientation was available for review.
Plan of Correction: Staff will obtain training.
