Inspector Notes

An unannounced monitoring inspection was conducted on 11/8/2023 from 11:00 am to 12:15 pm. There were 26 children in care, ages two and a half years to five years, in two groups under the care of five staff. The Licensing Inspector reviewed three children?s records, three staff records, and other program records. The children were observed on the playground and in quiet activities.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to obtain a TB assessment on a new staff person that was completed within 30 days prior to employment.

EVIDENCE:
1. Staff 2 has a documented start date of 9/25/2023 and the TB screening was dated 9/27/2023.

2. Staff 3 has a documented start date of 9/26/2023 and the TB screening was dated 8/12/2022.
Plan of Correction: From now on, TB assessments will be done prior to employment.
