An unannounced monitoring inspection was conducted on January 24, 2024 This on-site visit was conducted from 11:15 a.m. through 12:00 p.m. Eight children, ages 3-4, were in the care of two staff. Five children and Three staff records were reviewed.

Based on record review the facility failed to obtain repeat fingerprint and central registry checks for one staff.
Evidence:
1. Three staff files were reviewed.
2. Staff #3 did not have documentation of obtaining repeat fingerprint and central registry checks. Staff #3 last documented fingerprint check was dated 09/15/2024. Central Registry check was dated 07/10/2018.
