Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 02/16/2022 and concluded on 02/17/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 136 children present with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence: 1) The record for Staff #1, hired on 11/15/21, had documentation of a sworn statement that was dated 08/26/19 and documentation of a central registry finding that was dated 09/27/19. 2) During interview, a member of management reported Staff #1 was previously hired at the center and a new sworn statement or central registry finding were not obtained upon rehire. The staff member was separated from the center for more than 12 consecutive months.
Evidence: 1) The tuberculosis (TB) screening results in the record for Staff #2, hired on 08/26/2021, were dated 09/26/2021. 2) The tuberculosis screening results in the record for Staff #4, hired on 09/17/2021, were dated 10/10/2021. 3) During interview, a member of management acknowledged the TB results for Staff #2 and Staff #4 were not obtained within 21 of beginning employment. Staff hired prior to 10/13/2021 were required to obtain TB results within 21 days of beginning employment and such documentation was to be retained in the staff record.
