Inspector Notes

An unannounced monitoring inspection was conducted on Wednesday, January 24, 2024, to determine the center?s compliance with licensing standards. The inspection began at approximately 9:20am and concluded at 12:45pm. There were 130 children present with 11 staff supervising. The inspector observed the children playing on the playground, transitioning to centers, and playing in the gym. The inspector reviewed compliance in the areas of administration, physical plant, staffing, and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard 8VAC20-780-270-A
(Repeat Violation) Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
1. In the PreK II classroom located in the afterschool building, there was a child size red chair that had torn material that was accessible to children.
2. In the toddler?s classroom, there were soft blocks that were torn on the edges exposing the inside materials presenting a choking hazard.
Plan of Correction: Tears will be taped and new items ordered.
Standard 8VAC20-780-570-E
(Repeat Violation) Based on observation, the center did not ensure to label infant bottles containing prepared infant formula with the child?s name and date.

Evidence:
1. In the infant I classroom, the inspector observed six infant bottles containing prepared formula that were labeled with names but was not labeled with the date.
2. In the Infant II classroom, there were 15 infant bottles containing prepared formula labeled with names but was not labeled with the date.
Plan of Correction: Bottle Policy will be reviewed with all staff and date stickers will be checked by leads daily.
Standard 8VAC20-780-60-A
Based on review of ten children?s records, the center did not obtain all required documentation for four children?s records.

Evidence:
1. The record for child #1, did not contain the name and phone number of the child's physician. Child #1?s enrollment date was 1/30/2023.
2. The record for child #4, did not contain the work phone number for two parents listed and did not contain the name and phone number of the child?s physician. Child #4?s enrollment date was 8/1/2022.
3. The record for child #8, did not contain the work phone number for two parents listed, and did not contain the name and phone number of the child?s physician. Child # 8?s enrollment date was 8/21/2023.
4. The record for child #10, did not contain the parent?s work phone number. Child #10?s enrollment date was 8/21/2023.
Plan of Correction: Updated applications will be completed and added to each child's file.
