Inspector Notes
An unannounced inspection was initiated on March 29, 2024 in response to a self-report that was received by the licensing department on March 12, 2024, regarding an allegation of inadequate supervision. On March 29, 2024, the inspector was on site from approximately 10:00 AM until approximately 11:45 AM. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation does support the allegations therefore, the self-report is determined to be valid. Violations were cited.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
1Evidence: Child #1 and child #2 were in a back office unsupervised for approximately 8 minutes per staff and the parent of child #1.
On March 11, 2024, at approximately 4:30 PM, staff #1 and staff #2 transitioned the toddler class from the playground back to their classroom. It was reported that staff did not complete name to face check when entering the toddler classroom. The parent of child #1 (18-month-old) entered the classroom to find that the child was not in the classroom. Staff #1, staff #2 and the parent of child #1 began searching the building for child #1. When staff were unable to locate child #1, they notified an administrator and other staff. At approximately 4:40 PM, child #1 and child #2 (18-month-old) were found in a back office inside of the preschool.
