Inspector Notes

An unannounced inspection was initiated completed on 6/20/2024 in response to a complaint received by the licensing office on 6/10/2024 relating to allegations of staffing and supervision, and the physical plant. This inspection included interviews, tour of the facility, and document review.

The preponderance of evidence gathered during the investigation supports some of the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

IF VIOLATIONS ARE CITED, ADD THIS: Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
(Repeat Violation) Based on observation and interview, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The girls restroom door located in the school age classroom, does not open properly causing children to become stuck when trying to exit the restroom.
2. Staff stated that they are aware that the door jams at times. Issue has been reported to the upper management team.
